Richard is a rhinologist with particular interest in endoscopic sinus and medial skull base surgery. He trained in Auckland, the UK and Australia, and practised as a clinical immunologist and allergist at Auckland Hospital before undertaking training in ORL. He subsequently spent a fellowship year in rhinology in Adelaide.

His research interests include the role of bacteria in the pathogenesis of chronic rhinosinusitis, and postoperative interventions and clinical outcomes after sinus surgery. He heads a research team with several doctoral fellows, a post doctoral scientist and technicians based in the Department of Surgery of The University of Auckland.

Chronic nasal conditions are extremely common and cause enormous morbidity in our society. It is possible to alleviate most patients’ symptoms. The key is a simple, rational approach to the diagnosis and management of these conditions.

The important points of the history, examination and investigation will be discussed. A stepwise management protocol will be presented involving drugs, immunotherapy and surgery. The relative merits of these therapeutic options will be discussed.

A joint session with Jan Sinclair, paediatric immunologist and allergist, in which the practical details of administering sublingual and subcutaneous immunotherapy will be discussed.

Desensitisation for airborne allergens can be a highly effective intervention, but it is not without risk. Consideration of who, how, when and where patients can be considered for desensitization will be discussed.

Epistaxis is a common and troublesome event which can very occasionally be life threatening. The majority of cases can be effectively managed in general practice. The techniques for managing anterior and posterior epistaxis will be discussed.

An extraordinary range of pathologies affect the nose and sinuses, but it is not an area that is often examined. Simple techniques of obtaining a view into the nose will be demonstrated, and the diagnostic findings common in general practice discussed.
