Vanessa Brown
Vanessa has been a pharmacist since 1999, with a variety of clinical pharmacy experience in Hawkes Bay and UK. While working in the UK, Vanessa became involved in Palliative Care Pharmacy and in 2002 when returning to New Zealand took up a role as a Palliative Care Pharmacist at Cranford Hospice in Hastings. Vanessa completed a Post Graduate certificate in palliative care through Flinders University in 2006.

In 2012 Vanessa joined the Clinical Facilitator team at Health Hawkes Bay/Hawkes Bay District Health Board. Vanessa’s role is focused on patients 65 years or older living in their own homes and taking 5 or more long-term medication. This role is 0.5 FTE and is addressing medication issues relating to poly-pharmacy in the elderly.

Vanessa continues to work 0.5 FTE in palliative care and has found great synergy between the two roles as we face the challenges of an aging population and patients with multiple co-morbidities.

Vanessa was previously chair of the palliative care medication working group which reported to the Ministry of Health and continues to work with the palliative care advisory group on medication related issues.

Hawke’s Bay has implemented three 0.5 FTE clinical pharmacists into volunteer general practice teams to facilitate evidence based prescribing in primary care.

Three different models were trialled to see how this service may be best delivered over a 2 year pilot.

All three sites showed a significant benefit from having a clinical pharmacist as part of their general practice team. These results will be discussed along with how the service was implemented and the experience of those involved.
